Order Policy

Patient Supplement Order Policy

  • For an order to be placed, an Order Form must be filled out and submitted to the office via email or fax. No order will be taken over the phone.
    • An order must be placed for items added to an updated regimen, both when it is updated at an in-office visit and virtually. 
    • For patients at an office visit: after the visit is complete, the patient receives an updated regimen. For any supplements on this regimen that the patient needs to purchase, but the office is out of stock, an order form must be filled out. This is to ensure the patient receives their needed supplements. 
    • For patients that had a virtual visit or phone consultation: after the visit is complete, the patient receives an updated regimen. For any supplements on this regimen that the patient needs to purchase, an order form must be filled out. This is to ensure the patient receives their needed supplements. 
  • The office has 10 business days to complete a supplement order. No order will be completed within the same day of placement. Please do not request this. 
    • It is the patient’s responsibility to ensure that they have enough supplements until their order is received.
  • Patients will be notified when their order is ready for pick up. Orders can only be picked up on specified pick-up days and times, unless otherwise arranged with the office.
  • All orders must be paid in full upon pickup. Check is preferred so payment can be prepared in advance. 

Order Pick-Up Days and Times:

Mondays: 4:00pm –  6:00pm

Fridays: 10:30am – 1:30pm
