Dr. Holder sees new patients on Wednesdays from 8 am to 11 am, Thursdays from 3 pm to 6 pm, and every other Saturday from 8 am to 12 am.
Our Comprehensive Holistic New Patient Intake (CHNPI) is separated into TWO VISITS.
FIRST CHNPI VISIT fee is $295-395. It will last at least 45 minutes and consists of a medical and nutritional consultation with Dr. Holder.
SECOND CHNPI VISIT fee is $295-395, which is partially billable and occurs approximately 2 – 4 weeks after the first visit. It will last at least one hour and is conducted entirely by Dr. Holder.
During the intake process, Dr. Holder will gather the information that he needs to assess the patient’s whole being.
Subsequent visits are typically follow –up visits. Fees range from $85-295, depending on the duration and services rendered. During a follow-up visit, oftentimes, patients meet with the nutritionist first, and then Dr. Holder meets with the patient to discuss their case. He may give further recommendations or tell the patient to continue on their regimen.
INSURANCE – Please note that the only insurance provider that we accept is Medicare. After fees for services rendered are paid, the medical receptionist will gladly provide you with an insurance claim form to submit to your carrier for reimbursement. Please note that approximately HALF of the new patient intake is billable.
CANCELLATION POLICY – Please note that our cancellation policy requires 48 hours notice for cancellation of an appointment. If 48 hours notice is not given, you will be charged $85 for a missed 30 minute appointment and $125 for a missed 1 hour appointment. Please note that this policy is strictly enforced.
