Patient Supplement Order Instructions 

  1. Patient fills out a Supplement Order Form: to place a supplement order, a patient must fill out a Patient Supplement Order Form which is supplied by the office.
    1. Filling out the form: There are two ways to fill out the order form –  electronically or by hand. PLEASE FILL OUT THE FORM AS COMPLETELY AS POSSIBLE.
      1. Electronically: The office will send the form via email as a word document. The recipient can then download the form and fill it in on the computer. After the form is filled in, save the document.
      2. By Hand: If the patient is unable to fill out the form electronically, it can be done by hand. There are two ways:
        1. A patient can pick up a printed supplement order form from the office OR 
        2. The office can send the form via email and the patient can print it out at home. 
  2. Patient’s Supplement Order is Placed: A Patient Supplement Order can be made via email or fax. In cases where the patient is unable to use these two modes of ordering, the form can be dropped off to the office.
    1. By Email: To place an order via email, please follow the instructions outlined below. 
      1. Email Address: [email protected]
      2. Subject line: in ALL CAPS, make the subject: SUPPLEMENT ORDER.
      3. If the form was filled out electronically, attach the document to the email.
      4. If the form was filled out by hand, take a photo and attach it to the email.
      5. By Fax: To place an order via fax, send the form to this number:

  1. Order Is Received By Office: When the order is received, the office will contact the patient to confirm that it was received. Please allow up to 3 business days to receive a reply. The office will reply by email primarily and by phone if email is not an option. 
  2. Order Is Prepared: The order is then prepared. Please allow up to 7 business days for the order to be prepared and ready for pick up. Please order at least two weeks before running out of any supplements. This is to ensure the patient is not without their supplements.
  3. Order Is Ready For Pick-up: When the order is ready for pick up, the office will contact the patient. Upon contacting, the office will provide the patient with their order total. After being contacted, the patient can come at a designated day and time to pick-up their order.
    1. Pick-up Day and Time:
      1. Mondays: 4:00pm – 6:00pm
      2. Fridays: 10:30am – 1:30pm
  4. Payment: Payment is collected at time of pick-up. The preferred form of payment is check, but credit and debit card is accepted as well.

NOTE: THE OFFICE CAN NOT GUARANTEE THAT ALL SUPPLEMENTS WILL BE IN STOCK AND AVAILABLE AT TIME OF ORDER PLACEMENT. THE PATIENT WILL BE INFORMED OF ANY SUPPLEMENTS THAT ARE UNAVAILABLE.
