WHAT CPM OFFERS MEDICARE PATIENTS
- Welcome to Medicare Visits
- Once in a lifetime visit
- Must happen within the first 12 months after effective date of the Part B Medicare Plan
- A simple physical specifically for Medicare patients
- Initial Annual Wellness Visit
- Once in a lifetime visit
- Available for all Medicare beneficiaries who are no longer within the first 12 months of Part B Medicare Plan
- Available to those who have not received a Welcome to Medicare Visit within the last 12 months
- Evaluation/Exam is the same as the Welcome to Medicare Visits
- Annual Wellness visit:
- The subsequent visit after either the Welcome to Medicare Visit or the Initial Annual Wellness Visit has occurred.
- Visit can happen once a year for the rest of patients lifetime
- Evaluation/ Exam is the same as the Welcome to Medicare Visits
- Face to Face Behavioral Council for Obesity
- Available for Medicare Beneficiaries with obesity.
- One visit every week for the first month.
- Once visit every other week for months 2-6.
- At the six month visit, a re-assessment of obesity and a determination of the amount of weight loss must be performed. To be eligible for additional face-to-face visits occurring once a month for an additional 6 months, patient must have lost 6-7 pounds to continue.
- One visit a month for months 7-12
- Face to Face Behavioral Council for Obesity
- For those that do not lose the desired weight (6-7 pounds) during the first 6 months, a reassessment of their readiness to change and current BMI (body mass index) is appropriate after an additional six month period.
